Luke specifically asks her about her "real mother" so I have to say it's your answer that makes little sense. The death of her foster mother is not mentioned in any of the films so we, as an audience, have no way of knowing that she died when Leia was a "toddler". I have not seen Rogue One so I'm just going off what's in the main films.

I think this is what the dialogue between the two of them is trying to illustrate, that Leia's ability to remember her mother, despite dying when she was "very young" is a sign in itself that Leia is strong with the Force. Someone can have an affinity with the Force without being a Jedi, which is why Padme dying when her children have just been born is not a flaw in the story-telling. Leia's natural affinity allows her to still retain memories of someone after they have died - it's no different to the connection that Luke has to Obi-wan after his death.

What I wonder is: how is Luke aware of her being adopted? who told him?

    I think it's this.

If one refer only to the movies, for my part the only valid way is that Leia speaks of Breha Organa, and Breha Organa died when Leia was young, and at this very moment when Luke asks her, she thinks his mother always was Breha. Like some others here, I think any newborn can remember his first hour in life. I also don't subscribe to "the force" when there is no more way possible.
But. If this is the case, if Leia speaks of Breha and is convinced that she is her mother, that leads to another issue: why is not Leia surprised when Luke says "real mother" ?

Probably Leia. Between Episode IV and V they spent quite a lot of time together.

Makes you wonder how much of an oversight I may have been or alternative endings Padme may have had.

Queen Breha was killed when the Death Star destroyed Alderaan. In Leia's conversation with Luke, she told him that her memories of her "real" mother were basically images or feelings. I doubt very much that she was describing Breha, who had died four years earlier.

I voted Breha initially, but the more I sit here and ponder on it...it makes more sense to me that it would actually be Padme that she's remembering. She was a Force sensitive, despite not being trained. That alone is explanation enough to me as to how she can recall Padme. The piece of dialogue that makes me think it's Padme is Leia saying she was, "Kind, but sad." That sadness would've likely been the last emotion Padme felt. Even knowing that there was still good in Anakin, or there was still Anakin in Vader, I should say. She lost the love of her life and knew that their children would never get to know their father as the great hero that he had been to the galaxy.

I always assumed she was remembering her adopted mother. Which is why I always thought the flap about her not being able to remember Padme was a bunch of bunk.

    Pure....undiluted.....bunk. It's bunk.

SCENE: ENDOR MOON EWOK VILLAGE. NIGHT.

LUKE: "Leia, do you remember your mother? Your real mother?
LEIA: "She was very beautiful.... coarse.... and itchy."
LUKE: "You have nightmares about sand as well?"
LEIA: "Just feelings, coarse sandy ones. I can't explain it."

*both sigh and scratch itches together*

LUKE: "Leia, do you remember your mother? Your real mother?
LEIA: "I shouldn't because she died literally a minute after she gave birth to me."
LUKE: "I remember. But I was first."
    LEIA: "No you weren't"
    LUKE: "Was too."
    LEIA: "WAS NOT!"
    LUKE: "WAS TOO!"

Padme in what is arguably a self inflicted continuity error though I'll grant we can hand waive it with force memories.

That doesn't fix the actual lame death for weak dramatic effect but that is left maybe for a different topic and day.
