    The SW universe in relation to mechanical things demonstrates that wear and tear occurs and items need upgrading or replacing.

    Luke's hand in vision being different to the one seen at end of film could be a result of this,plus the vision might be of the past or it may be of the future.

    I'm going with it's the same hand and he stopped replacing the synthetic skin years ago, or he's had various hands over the years. Either way I don't think it will be a plot point or need any explanation. In fact, I don't WANT every little thing explained. A little mystery is a good thing.
